Program Coordinator - UTOP

Job Summary

Community Outreach Specialists
The UTOP Tele-Retina Program is a telehealth initiative focused on increasing access to diabetic retinopathy screening and eye care services in undeserved communities. The Program Coordinator will play a critical role in expanding the program's reach by building relationships with local healthcare providers and coordinating follow-up care for patients identified with retinal disease.

Responsibilities
- Provider Engagement & Outreach
Identify and reach out to local primary care clinics, optometrists, and health systems to participate in the tele-retina screening network.
Develop and maintain relationships with community providers and clinic leadership to ensure sustained participation and collaboration.
Conduct informational sessions and presentations to educate clinics and providers about the benefits of the UTOP Tele-Retina Program.
- Care Coordination & Patient Follow-Up
Collaborate with Federally Qualified Health Centers (FQHCs) to establish referral pathways and ensure timely treatment for patients with identified retinal conditions.
Coordinate appointments for specialty care and assist with navigation to appropriate follow-up services.
Track patient outcomes and communicate updates to referring providers and care teams.
- Program Administration
Maintain accurate and up-to-date records of outreach activities, clinic participation, and patient referrals.
Assist with scheduling, data entry, and communication workflows between screening sites and the retinal specialists reviewing the images.
Support quality improvement initiatives and data collection to evaluate program effectiveness.
- Collaboration & Communication
Work closely with the UTOP clinical and outreach teams to align program goals and share feedback from community partners.
Facilitate meetings, training sessions, and ongoing communication with participating providers and clinic staff.



Minimum Qualifications
EQUIVALENCY STATEMENT: 1 year of higher education can be substituted for 1 year of directly related work experience (Example: bachelor's degree = 4 years of directly related work experience).
Department may hire employee at one of the following job levels:
Community Outreach Specialists, III: Requires a bachelor's (or equivalency) + 6 years or a master's (or equivalency) + 4 years of directly related work experience.
Community Outreach Specialists, IV: Requires a bachelor's (or equivalency) + 8 years or a master's (or equivalency) + 6 years of directly related work experience.

Preferences
Strong interpersonal and communication skills, with experience in community outreach or provider engagement.
Knowledge of healthcare systems, care coordination, or telehealth services preferred.
