Essential FunctionsPatient Navigation:
- Serve as a bridge between the community and the dental clinic, helping refugee and other vulnerable population patients access dental services.
- Assist patients with appointment scheduling and transportation arrangements.
- Assist patients with navigating financial resources and assistance programs.
- Provides timely documentation into database of all patient interactions, interventions and community resource contacts and communicates contacts to dental physician.
Health Promotion and Advocacy
- Advocate for the oral health needs of the community, especially for refugee and other vulnerable population patients facing barriers to care.
- Identify systemic issues that affect oral health access and work with the clinic to address them.
- Distribute educational materials on oral hygiene, nutrition, and preventive care both in clinical and community settings.
Referrals and Follow-Up:
- Provide wrap around service to help patients move between integrated medical and dental clinics and assist patients in making appointments at outside clinics as needed.
- Follow up with patients to ensure they complete recommended treatment plans.

Minimum Qualifications
High School diploma, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated relationship-building and effective communication skills.
This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.
Care is appropriate to the population served.
Applicants must demonstrate the potential ability to perform the essential functions of the job as outlined in the position description.
Preferences
Previous experience working with refugee populations strongly preferred.
