Job Summary
Coordinating Access in Complex Care
The Intensive Outpatient Clinic (IOC) serves individuals with complex medical, behavioral, and social needs. Many of our patients face barriers related to transportation, housing instability, food insecurity, and maintaining consistent care over time.
The Patient Relations Specialist serves as a central point of contact for patients, coordinating with the care team, facilitating access to services, connecting resources, and addressing barriers that affect care continuity.
This is not a traditional front desk position.
This role is central to how our clinic functions.

Work Environment
This is an on-site position located at the University of Utah Population Health Center near Rose Park in Salt Lake City.
Nearly continuous office-based work including sitting, speaking, listening, and computer use.

Responsibilities
Essential Functions:
The PRS supports patients and the interdisciplinary care team by ensuring smooth access, accurate information flow, and coordinated follow-through. Core responsibilities include:
Patient check-in across IOC and Population Health clinics (including visit type and insurance verification) Scheduling and rescheduling follow-ups, recurring visits, groups, nurse visits, PT, and dental appointments Managing phone encounters and accurately routing clinical and administrative needs Coordinating transportation through Modivcare (TripCare portal, medical necessity forms, same-day changes) Managing appropriate use of rideshare services under clinic protocols Verifying insurance eligibility and guarantor information Supporting new patient onboarding and minimum data collection Collaborating closely with medical assistants, nurses, providers, and care coordinators Monitoring patient flow and communicating changes to the care team Supporting access to interpreters (iPad-based and phone services)
This role also supports practical patient needs that reduce barriers to care, including: Coordinating food pantry deliveries and documentation Supporting hygiene and clothing closet access Assisting with outreach efforts and referral verificationMaintaining inventory and clinic organization
Minimum Qualifications
- One year of office experience, customer service experience, or equivalency (one year of education can be substituted for two years of related work experience).
- Current, valid Utah driver's license at time of hire may be required in some areas.
- Strong written and verbal communication skills
- Demonstrated human relations skills
- Ability to maintain confidentiality and adhere to HIPAA regulations
This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.
Preferences
- Associate degree in business or related field (or equivalent combination of education and experience)
- Three years of full-time secretarial or administrative experience
- Office or customer service experience
- Knowledge of insurance verification processes
- Experience coordinating transportation services
- Valid Utah driver's license (if required for certain duties)
