Responsibilities*
Nursing:
  • Triage and review unclear referrals to Cardiology, Cardiac Surgery, and Vascular Surgery ensuring patients are scheduled with the right physician
  • Triage unclear referrals to bridge clinic:
    • Subset of patients who should be seen by a cardiologist post discharge instead of APP due to complexity
    • APP bridge for cardiology, arrhythmia, and heart failure-ensure patient is placed in the correct bridge
    • Ensure patients with non-cardiac related events do not get placed in our bridge and help coordinate appropriate care for patient
  • Assist with Cardiology Access: Determining appropriate placement into APP verse Cardiologist grid.
  • Review records and assist with appropriate placement of patients
  • Serve as a resource to the call center with patients who call in with symptoms and the call center agent is unclear of whether a red call should be implemented or if the patients' needs are urgent.
  • Assist with verifying appropriate pre/post appointment orders are in place.
  • Assist with transition of patients from inpatient to outpatient setting.
  • Coordinate care with call center agents and primary nursing staff as needed.
  • Provide instruction and consultation to call center representatives as appropriate, regarding clinical questions.
  • Triage incoming referrals in order to 1) facilitate scheduling the most acute patients as soon as possible and 2) ensure that patients are seen in the correct program and by the most appropriate provider.
  • Participate in the ed call process by phone assessment and coordination of care for patients experiencing urgent medical symptoms.
  • Additional task as needed to support nursing work within the CVC clinic.

Team Liaison:
  • Be a resource for the call center agents
  • Assist with training and education for the call center agents as it relates to clinical content
  • Resource and support RN staff at the following clinics/ACU: CVC, Disease Management, DF Cardiology and additional Cardiology areas as a necessary.
  • Assist with the development and updating of orientation materials for RN role and for call center agents
  • Meets with Call Center Leadership team monthly and participates in planning resources, development, and support.
  • Provide in-services for the call center employees

Salary & Nursing Framework Level:This UMPNC RN posting is posted as Nursing Framework LEVEL C.Actual Nursing Framework LEVEL and salary will be determined at time of hire.Nursing Framework levels range from Level A to Level F.

Salary range for Registered Nurses- $65,840.24 - $104,150.85

  • Evening Shift Differential- $2.45 per hour

  • Night Shift Differential- $3.30 per hour

  • Weekend Differential- $1.75 per hour

    Required Qualifications*
    • Current license to practice nursing in Michigan is required.
    • Charge nurse experience
    • Two years' recent ambulatory experience including telephone triage (must be within the last five years).
    • At least 3-5 years' experience working as a registered nurse.
    • At least 3 years nursing experience with the cardiovascular patient population.

    Desired Qualifications*
    • Bachelor of Science in Nursing
    • Interventional cardiology experience
    • Three or more years of experience in ambulatory with telephone triage
    • Greater than 3 years' experience with the cardiovascular patient population
    • Experience with MiChart (EPIC) and Aspect.
    • Demonstrated knowledge and experience with continuous quality improvement
    • Desire to participate on committees and projects to improve patient outcomes
    • Demonstrated ability to work collaboratively and effectively with a multidisciplinary team with the goal of service excellence and Patient & Family Centered Care.
    • Willingness to perform the role with excellent multitasking abilities.
    • Excellent competence in verbal and written communication skills, organizational skills and problem-solving skills.
    • Demonstrated ability to communicate assertively and effectively and act as the patient advocate.
    • Demonstrated high level of self-motivation to learn and stay current in the treatment of cardiovascular patients. Self-motivation to participate in activities to improve patient care.
    • Demonstrated ability to positively contribute to a "team" approach to patient care.
    • Demonstrated clinical expertise in cardiology practice including:
      • Diagnostic test interpretation including EKG interpretation of abnormal rhythms
      • Cardiac medications
      • Emergency management of cardiac patients in an outpatient setting
      • Familiar with ACC (American College of Cardiology) guidelines for general cardiology patients
      • Lipid and risk factor management and secondary prevention
      • Evaluation of lab abnormalities;
      • Ability to assess patients mental, physical and psycho-social well-being over the phone and develop a plan of care without seeing the patient

    Work Schedule

    Hours: 40 hours per week
    Shift: Mon- Fri, 8 am- 5 pm
    Location: North Campus Administrative Complex (NCAC) and CVC, however location could vary to any ambulatory sites where cardiovascular services are present. All new employees will be expected to float to designated units in times of low census
